New Employee Forms

Several forms will require completion on or before your start date in order for your employment to begin and for you to receive your first paycheck. After you have accepted your employment offer, you are required to complete and sign all documents listed below: 

 

Form Description Additional Required Forms/Documents
Direct Deposit Authorization of Employee Pay Authorizes your financial institution to receive and deposit your bi-weekly pay check. - Copy of a void bank check; or accounting or routing number from your financial institution. 
Employee Eligibility Verification (I-9) Verifies the identity and employment authorization of the new employee. Please refer to "Instructions for Form I-9. Employment Eligibility Verification. 
W-4 - Federal Witholding Authorization Identifies the number of personal withholding allowances for year-end federal tax calculations. N/A
K-4 - Kansas Withholding Allowance Certificate  Identifies the number of personal withholding allowances for year-end federal tax calculations. N/A
